The scenario is, where I witnessed my friend Ling Ling prepare the dressing set for wound dressing not following the aseptic technique. My clinical instructor Rosy is not aware of her contamination. I am in a situation on how to convey this incident to her. The ethical dilemma; I should not inform CI Rosy about what I saw earlier, as I am also a student and Ling Ling is my friend. Thus I should not speak about this to my clinical instructor and get Ling Ling into trouble. The second option would be to be truthful enough to inform CI Rosy about what I saw earlier as this is the right thing to do. However, my friend Ling Ling would be in trouble for not following aseptic technique, and this would reflect poorly on her conduct and may even affect her grades for the posting.Identify ethical dilemma & Pertinent data Student nurse Ling Ling did not perform the surgical hand rub correctly. Secondly she dropped the forceps from the dressing set, and decided to pick it up with her finger. She continued to place the contaminated forceps in the dressing set, which was a sterile field. She also winked at me, to signal me to keep it between us. The purpose of performing the surgical hand rub is to remove microorganisms and reduce the number of bacteria colonies forming in the hands. According to the world health organizations(WHO), there are thousands of people who die every day in the world due to hospital acquired infections (1). Our hands are a route transmission for germs to travel from one place to another. The best way to prevent direct germs transmission is to perform proper hand hygiene. As stated by world health organization, hand washing could decrease diarrheal disease-associated deaths by up to 50%(“Hygiene Fast Facts| Hygiene | Healthy Water | CDC”, 2018). Researchers in London have found out that if everyone follows the proper technique of hand hygiene, a million deaths per year could be prevented (“Hygiene Fast Facts| Hygiene | Healthy Water | CDC”, 2018). Other researchers have stated that 80% of communicable diseases are transmitted through hand contact((“Handwashing saves lives”, 2018)). Maintaining aseptic technique will help to decrease the chances of complications to the patient. When the aseptic technique is bridged, patients may be at high risk for developing surgical site infections (“Aseptic Technique: Uses, Benefits, and Complications”, 2018). Even the slightest contact with patient can transmit 10 000 colony-forming units to hands (Gould 1993) (“Aseptic technique”, 2018). Intergrity is one of the key values a nurse should practice in healthcare at all times. When a nurse faces challenges, intergrity would be the principle that would keep one standing tall. Being honest in words and actions reflect on the attitude of a nurse. Examine dilemma for ethical principlesFirstly, the ethical principle fidelity has been bridged in this scenario. The act of performing care for patients with compassion and commitment has been compromised. As a health care professional, Ling Ling should take the responsibility given to her seriously on giving the patient the best of her care. Secondly, the principle veracity is not supported with her actions. Ling Ling dropped the forcep on the ground and placed it back on the sterile field. This behavior shows that she was not bothered about the outcome of using a contaminated set on the patient.The act of her negligence was proven,when she winked at me. This action of hers confirms that she wanted to hide her actions from CI Rosy. Thirdly the principle beneficence is bridged through her incompassionate act towards the patient. Beneficence is about the positive action of providing a helping hand for the patient to relief or reduce their pain. Ling Ling failed to help improve patients condition, by using the contaminated dressing set and performing improper surgical handrub.Possible solutionsThere are various ways in handling this situation. The first possible solution would be me confronting Ling Ling and explainning to her the mistakes she did during the procedure , afterwhich I should apologize to CI Rosy for not informing her on what I saw earlier. The second possible solution could be, me not informing CI Rosy about what I witnessed, but I should tell Ling Ling her mistake and advise her to not carry out any procedure with a contaminated dressing set, as it could put the patient at risk of developing infection at the wound site. Thirdly, I should just spill the beans to CI Rosy about what happened despite knowing that Ling Ling might be confronted by CI Rosy.  I personally feel that the most appropiate solution would be to inform CI Rosy about what I witnessed despite knowing that Ling Ling will be confronted and questioned. I should let the clinical instructor handle the situation as she would be more wise with her approach to her students. Even though my relationship with Ling Ling may be affected, it would be benificial to Ling Ling, as she will realize the importance of following aseptic technique. She will also be reaffirmed by clinical instructor Rosy about the possible consequences the patient may end up with. As a student, my words and my approach may give the impression to Ling Ling that I am trying to be a role model student. Therefore, this solution would be the best approach. It is necessary for me to do what is right, despite Ling Ling being my friend or not.  I feel that Ling Ling was not truthful in her actions. Being honest is a critical value a nurse should inhibit. If Ling Ling is negligent in this situation, she may continue on with her behaviour in the future as well. For example, Ling Ling may perform false charting of parameters, as there would be no one watching her all the time. This can put the patients life in danger.  Integrity is a core value that is required by everyone in the healthcare profession. Patients trust us in our actions when they are ill. Moreover as we are the first line caregivers in the hospital for our patients, we have to be careful with our actions. Argument structureAccording to the code of ethics and professional conduct value statement 5.2, it is the duty of the nurse to ensure the care of patient is not jeopardised at any cost. As a nurse, we should provide out utmost care for the patient. The basis in caring for a patient would be to follow the protocols of the hospital and maintain proper hand hygiene. Jeopardising the hand hygiene when performing the procedures could result in the patient developing severe infections and other health complications. Secondly, using contaminated equipments for a sterile procedure is a miconduct of the nurse.    According to the second value statement 8.1, it is necessary that the care given to patient does not result in harm or endanger the standards of care they receive. Ling Ling has bridged the care of duty as a nurse, as she has placed her patient at risk for developing other infections or complications due to her negligence in aseptic technique. As nurses, we are supposed to provide the best of quality and care for the sick, in the means of improving their health condition. However, Ling Ling has failed to keep up her role as a health care professional as she has compromised her infection control practice. This act of hers could possibly increase patient’s stay in the hospital. Overall, all of these could incur into the patients finance. According to the standard 2 in the standards of practive for nurses and midwives, nurses demonstrate professional conduct and practice in accordance with the standards of practice, as patients trust nurses with their wellbeing(“standards of practice for nurses and midwives”, 2018). Therefore the nurses have a duty to sustain the standards of practice and respect patients life, dignity and care for their welfare.  Ling Ling is aware that she has to be proficient when handling patients. As nurses we must remember to preserve patients diginity and also respect their life. In conclusion, I would inform my clinical instructor about what happened inside the curtains while she was away. I feel that this is the best solution as my clinical instructor will handle Ling Ling personally. I do know that through this, my relationship with Ling Ling may be affected. However I hope this would help her be a better person, and also prevent her from doing anything that would bridge intergrity. Intergrity is a important value that every nurse should possess. Being honest will always save one from any trouble and thus we do not have to fear anyone if we do what is right at at all times.
